A specific TCI (‘To Come In’) policy exists for the medical specialties to guide appropriate admission (see appendix 3).
Admissions to the specialties should be discussed with the on-call staff but it is important to note that A&E have TCI rights throughout the hospital. However it is vitally important that this ‘power’ is not abused: admissions must be appropriate and not just convenient. If a decision is not clear or you encounter difficulty with colleagues to whom you are referring patients discuss the situation with a senior as soon as possible. Endeavour to remain calm and polite with your colleagues at all costs. This is not just simple courtesy but a requirement of the GMC!
Always remember to write a time of referral and time of TCI on the card. In patients who clearly need admission there is little point in waiting for the results of investigations. The relevant bleep numbers or telephone numbers to contact the on-call speciality staff are displayed around the department.
