Pharmacist
The Liver Pharmacist visits the Liver Unit every week day and deals with all medicine related issues. They also attend the post transplant outpatient clinic.
During the ward visit the Pharmacist will:
- Review the medication that each patient is prescribed to ensure they are taking the correct drugs in the correct way at the correct doses
- Ensure that there are no interactions between prescribed drugs and that the patient is not suffering from any unwanted side effects.
- Answer any questions that patients have in relation to their medication
If you are admitted to the ward you should always bring your medication in with you (including over the counter medication) so that the pharmacist can check what you are taking.
The Pharmacist also advises the doctors and nurses on the Liver Unit on any medication related issues including dosage advice, monitoring requirements and information on administration.
An important role of the Liver Unit Pharmacist is to give information to patients about their medication and to educate patients on new medication after having a liver transplant. This includes information on how and when to take the medication, common side effects and how to get further supplies.For further information about medication post transplant click here
A Pharmacy Technician also visits the Liver Unit each week day to ensure that each patient has a supply of medication to take while on the ward and to go home with. This includes checking patients’ own medication and ordering medication from the hospital pharmacy that is not stocked on the Liver Unit. When a patient is to be discharged from the Liver Unit, the pharmacy technician will ensure that they have enough medication to take home.
