How can I access the service?
Anyone within the Yorkshire area can be referred to the Leeds MND Care Centre. If you would like advice or information you can contact the care centre and speak to someone directly. If you would like to attend the clinic and be reviewed by the team you will need to be referred by your GP or hospital consultant. You should already have been given a diagnosis of MND. A letter needs to be sent to Dr Johnson the consultant neurologist outlining your symptoms and why you would like to be seen. The specialist nurse/care centre co-ordinator may contact you before an appointment is offered. If an appointment is needed this will be sent to you as soon as possible.
