How To Refer

Referrals to this unit can only be accepted from a GP or a Consultant. Therapists that wish to refer patients must have approval from the consultant caring for the patient before sending a referral.

Please complete the referral form and fax this form urgently FAO either
Professor Simon Kay, Mr Waseem Saeed or Miss Grainne Bourke on 0113 2066423 (Fax number only).

Alternatively you can mail the referral proforma to:

If you have any queries regarding the referral of these injuries please contact us on 0113 2067282.
Please note that all new referrals must be ideally sent as soon as possible post injury. This enables the child to be assessed at 12 weeks of age (please check the referral guidelines) and will ensure primary nerve surgery if indicated is performed early with appropriate strategy designs being formulated for each child. However, referrals will be accepted at a later stage.
