Guidance for researchers on accessing patient records
When requesting patients' notes please note the following:
-
You will need to provide separate lists of deceased and living patients
-
You will need to provide separate lists for LGI notes and St James' notes. LGI notes identifiers are 7 digits and St James' are 6 digits.
-
Health records are pulled in batches, so when requesting notes please only request those you can reasonably use in a 1 week time period
-
Clear instructions must be provided of where the notes are to be tracked to and contact numbers
-
There is a time restriction of 1 week on how long the records are allowed out of the main file, so please ensure that the notes are reviewed soon after they are pulled
-
There is a 24hr facility which enables notes to be viewed, though this has to be pre-arranged.
To request patients
notes you need to complete a general request for health records form, there
are separate forms for the LGI: LGI
Form
and St James' sites SJUH
Form
.
An automated response will be sent to notify that we have received your request.
Requests will be prioritised. The medical records personnel will fax back the
request form once the health records have been received. Please ensure your
fax number is on the request form. For those who do not have access to email
please contact the relevant health records department on the numbers below.
Once the form is completed you can either fax or email it to the appropriate location.

Health Services Research Workshop
The launch of the new Department of Health (DH) Research & Development Strategy has changed the way research in the NHS is funded. Research monies are distributed across several funding streams via the National Institute of Health Research (NIHR). The NIHR has several open competitive national funding streams that researchers must apply for. The key objective of the funding streams is to improve the quality, relevance and focus of research to the NHS and personal social services. The R & D Department is therefore held a workshop with the following aims:
Aims
- To offer support to clinicians, academics and researchers who are considering making a submission to these funding streams.
-
To establish collaborations between clinicians and academics to work jointly on developing submissions to the NIHR applied programme grants schemes
- To identify a series of programme grant proposals for submission to the NIHR applied programme grants schemes.
