Notes For Suppliers Of Loan Or Free Issue Equipment

In view of modified NHS guidelines, and advice from the MDA, the Leeds Teaching Hosptials NHS Trust can no longer accept loan or free issue equipment until the following requirements have been met (classified as follows either managed directly by Medical Physics for all electrical biomedical equipment or for all other equipment arranged by the Supplies Department):

The following information, once provided, will be stored by Medical Physics or Supplies. Subsequent loans, from the same supplier, will then only require the completion of a delivery note (This assumes that the insurance certificate is still valid).

  1. A valid IFA / IFB number is produced (MIA numers are valid until the expiry of the current insurance. Validity will be checked at http://www.pasa.doh.gov.uk/Purchasing/mia/)


    OR

  2. An appropriate indemnity form has been signed by two directors of the company (signatures of one director and one company secretary will also be acceptable).

    A copy of the current insurance certificate (minimum £5 million) is held by Medical Physics or Supplies.

    The original indemnity form and the copy of the insurance certificate should then be posted to Medical Physics or Supplies at the above address.

  • If any patient-contact accessories are provided proof of decontamination, or instructions for cleaning / sterilising, should be supplied
  • User instructions and service history of the equipment must be provided
  • All loan equipment must be acceptance, and safety tested by Medical Physics
  • Indemnity for the Loan, Trial or Provision of Free Electrical Equipment

    All loaned, trial or free electro-medical devices, equipment or accessories must undergo a full acceptance test performed by Medical Physics.

    As described above the NHS Form Of Indemnity must be completed in conjunction with Medical Physics or Supplies and signed by the responsible officers of the supplying company.

    Medical Physics will keep a register of all loaned, trial and free electro-biomedical equipment including its ownership, service history, current location, service responsibility and ensure that copies of the instructions for use and, where appropriate, for decontamination/sterilisation are available.

    All suppliers who have Trust authorisation to loan or trial equipment or when supplying equipment, free of charge must provide indemnity cover to protect the Trust.

    Exceptions to individual Indemnity Agreements are those suppliers who already provide a valid Master Indemnity Agreement (MIA) to the NHS.

    Pre-Purchase Questionnaire (PPQ)

    For medical (and some non-medical) equipment a PPQ may be required prior to purchase. This form relates to the technical specification of the equipment and will be approved by the Trust Medical Physics Department. The check ensures that the equipment complies with UK, European and Trust's safety standards.
